Summary Assume primary responsibility for completing admission process which includes the assessing, planning, implementing, and evaluating patient plan of care. Welcomes patient and assist with move-in process. Deliver Welcome Packet, answer questions, and begin C.A.R.E.S. process. Coordinate communication with the Family, Nursing Home Staff, Social Workers and team regarding the admission process. Complete initial plan of care in EMR. Complete head to toe assessment and ensure that all appropriate treatments and orders are in place. Add any wounds to weekly wound log. Initiate appropriate preventative measures (i.e., skin, fall risk, elopement, etc.). Initiate all appropriate admission paperwork including initial plan of care, med list, admit papers, Physician orders with appropriate dates and signatures, code status papers and other required documentation. Verify orders with physician. Obtain and enter into EMR patient's current immunization status. Offer / administer / document immunizations as appropriate. Demonstrate familiarity with policies of the agency and rules and regulatory requirements. May be assigned other duties as appropriate.
