Responsibilities

Coordinates the provision of hospice services for each patient and family

Direct and follow established policies and procedures as related to the scope of practice

Maintain continuity of patient care in a hospice setting

Proactive, collaborative team member in the hospice environment

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

• Current, active, and unrestricted Registered Nurse (RN) licensure in state of practice
